Should I attempt the speedo cable replacement on my ’90 V-6 4-Runner
with auto trans? The manual isn’t very clear – the pictures are of an
older model dash. Any helpful advice would be appreciated (like take it
to the dealer!).

I would say yes, replace it yourself. I replaced the speedo cable in
my ’89 V-6 4-Runner in about 20 minutes. Now that I’ve done it once
I think it would only take 10 minutes if I had to do it again. I do
have the manual transmission but I wouldn’t think the speedo cable
would be much different for the automatic.
