of his displeasure with what the Dims are doing to our healthcare.

Thank you for contacting me about health care reform. I appreciate hearing
from you on this important issue.
Health care affects every family and business in this country, and the cost
is simply unsustainable. Less than half of American small businesses can
afford to offer health care to their employees, and families have seen more
of their budgets go towards health care costs. The focus should be on
lowering the skyrocketing cost of health care, bringing down the high costs
of premiums for middle-class Americans, and ensuring Americans have access
to affordable, effective health care.
Unfortunately, the House bill before us contained provisions I could not
support. This bill imposes a drastic $502 billion in gross cuts to
Medicare. At the same time, this legislation also shifts more people onto
the Medicaid program, further straining state budgets that are already
facing deficits of at least $200 billion over the next few years. We cannot
expect doctors, hospitals and healthcare professionals to do more, with
less. This legislation costs too much. At a time when unemployment is over
10% and our national debt is approaching $12 trillion, I could not support a
bill that costs over $1 trillion but did nothing to ensure overall
healthcare costs will stop rising.
Presently, hospitals that can least afford to absorb further cuts are facing
the largest reductions in Medicare reimbursement payments under this
legislation. Our rural communities would be especially hard-hit, as
residency redistribution is not adequate for rural areas, reimbursements
remain too low, and the geographic makeup creates logistical challenges
metro areas do not face. This will not only mean a loss of jobs in the
district, but a loss of vital medical services for the entire region.
Pennsylvania’s hospitals rank third in the nation in efficiency, and we
should ensure our hospitals remain among the nation’s leaders.
I am concerned about the drastic cuts our nursing homes would be facing
under this bill. Financing this bill on the back of nursing homes to the
tune of cutting $32 billion in Medicare payments is unacceptable, especially
when this industry is already underpaid by Medicaid by $4.2 billion per
year. Additionally, proposed cuts to visiting nurses and the home health
industry will impose more stress on a system already strained. Our seniors
and disabled populations in need cannot afford to see these vital services
dwindle. To a largely non-profit industry operating on very slim profit
margins, facing the $300 million in losses that could result from this bill
would be devastating. In the 17th district, in 2010, 43% of home health and
visiting nurse organizations will be operating in negative profit margins,
escalating to 64% by 2011.

H.R. 3962 expands the qualifications for the Medicaid program, thereby
increasing those who qualify. This will be detrimental to states that
already cannot stretch their budgets to cover the current Medicaid
populations. It has been estimated that under this bill, without enhanced
federal funding and no change in reimbursement rates, Pennsylvania could
face over $8 billion in Medicaid costs from FY2013-FY2019. The 17th
district has over 37,000 beneficiaries currently choosing Medicare Advantage
plans for their coverage (which is over 30% of eligible Medicare
beneficiaries in the district). H.R. 3962 would impose $170 billion in
cuts to Medicare Advantage, meaning a loss of coverage and benefits, and
higher premiums and out-of-pocket costs.
I also believe it is important to remain fiscally responsible, keeping
America's deficits and long-term national debt under control. Currently,
health care costs account for eighteen percent of our gross domestic
product. Fiscally, the goal of any health reform should be to increase the
value of health care and slow the rate of growth in health costs. This
should include eliminating the one-third of the $2.3 trillion spent on
health care each year in duplicative and/or unnecessary payments. Health
care reform should create a system that saves money, or any reform will be
feeding the fire of an inefficient system. Commonsense reform is necessary
to bring down costs for everyone, while preserving what works in our current
system.
Finally, the conspicuous absence of both limits on illegal immigrants'
access to services, and meaningful tort reform in H.R. 3962 is troubling.
Unfortunately, there is no language to ensure illegal immigrants will not be
able to purchase coverage on the exchange - which is set-up using federal
tax dollars. While there is language ensuring they will not be able to
receive affordability credits, there is nothing to keep them from purchasing
a government-provided plan with their own dollars. Only the Senate Finance
Committee's version expressly exempts them from the mandate to have health
coverage. In addition, in a time when many Americans are losing access to
doctors, we need to limit the catastrophic malpractice premiums doctors are
required to pay.
I am hopeful Congress can continue our work towards a bill that includes the
elimination of penalties on preexisting conditions, closing the Medicare
prescription drug doughnut hole, allowing the purchase of insurance across
state lines, removing the cap on benefits, addressing the critical issue of
rising premiums, and ensuring seniors, young people, and families are no
longer faced with debilitating debt to stay healthy. Unfortunately, H.R.
3962 did not deliver these goals, and that is why I ultimately voted against
this bill. The United States Senate is currently debating and crafting its
own version of a health care reform package. I will continue to follow
this issue closely as both chambers of Congress proceed with legislative
action.
